Most Phase 2 trials are sized on a guess seen as mathematics. You are in the trial design meeting, your biostatistician pulls up a standard power calculation, someone picks an effect size that feels reasonable, say, a 30% improvement over placebo, and the software produces a sample size. The meeting ends. The number gets locked in.
